Recommended Networks include: BET, Bravo, Comedy Central, Discovery, Food Network, FX, Lifetime, MTV, TBS, TLC, TNT, TV Guide, TV Land, USA, VH1.
Tune-in spots must air 6am-11pm.

I hereby agree by and on behalf of my employer that my employer agrees to air a minimum of 300 E! Live from the Red Carpet® cross-channel tune-in spots from August 27, 2007 through September 16, 2007 (inclusive)*.
 
I hereby agree by and on behalf of my employer to complete any Cross-Channel Affidavit Form that may be provided by E! Entertainment Television, Inc. I acknowledge and agree that all Affidavits must be sent in no later than 30 calendar days following September 16, 2007, the final flight date for running the cross-channel spots. Submit your affidavits by going to www.eemmyextravaganza.com and log in using your Username and Password. Click on “My Account” and then click on “Submit your Affidavit”. Fill in the information and click on “Submit Online Affidavit Now,” or, alternatively, you can click on “Print & Return Affidavit Via Fax”. Print and sign the form, then fax the completed and signed Affidavit form to E! Networks, ATTN: Cary Yerg, (323) 954-2662. Once fully completed and executed Affidavits are received and approved by E!, online incentive merchandise orders can be released to participating and eligible systems. In the event that a participating system does not run the agreed upon minimum number of cross-channel spots from August 27, 2007 through September 16, 2007, (inclusive)*, the participating system hereby agrees to run "make good" spots as determined by E! Entertainment Television, Inc.
 
PARTICIPANT MUST AGREE TO ALL OF THE ABOVE CONDITIONS.
 
*Only Systems with 30,000 subscribers or more are eligible to participate
 
   
 
In the coming weeks, you will receive an email confirmation with a designated user id and password to begin redeeming your E! Red Hot Points. Taggable tune-in spots will be sent to participating systems in late July.
